BOARD OF PUBLIC WORKS AND SAFETY MINUTES October 7, 2014 Regular Session: The Board of Public Works and Safety met in regular session on Tuesday, October 7, 2014 at 9:00 a.m. in the Common Council Chambers. Members present were: Cindy Murray, Dawn Ross and Ron Shriner. Absent: Gary Henriott and Norm Childress Ed Chosnek, City Attorney, was also present. Mrs. Murray called the meeting to order. The Pledge of Allegiance was given to the flag of our Country. MINUTES Mrs. Ross moved for approval of the minutes from the September 30, 2014 regular meeting. Mr. Shriner seconded. Passed. NEW BUSINESS Engineering Contract-Sagamore Parkway Reconstruction, Phase II Jenny Leshney, Public Works Director, presented to the Board and recommended approval of a Contract for the Sagamore Parkway Reconstruction Phase II with Rieth-Riley in the amount of $10,850,000.00. Mr. Shriner moved for approval. Mrs. Ross seconded. Passed. Notice to Proceed-Sagamore Parkway Reconstruction Phase II Mrs. Leshney presented to the Board and recommended approval of a Notice to Proceed for the Sagamore Parkway Reconstruction Phase II for Phase 1 & 2 of the Maintenance of Traffic Plan. Mr. Shriner moved for approval. Mrs. Ross seconded. Passed. Certificate of Completion, Final Waiver of Lien and Guarantee-Twyckenham Blvd & Poland Hill Roundabout Mrs. Leshney presented to the Board and recommended approval of a Certificate of Completion, Final Waiver of Lien and Guarantee for the Twyckenham Blvd & Poland Hill Roundabout. The final amount is $1,450,814.65. Mrs. Ross moved for approval. Mr. Shriner seconded. Passed. Change Order #2-2014 North 18th Street Curb Replacement Project Mrs. Leshney presented to the Board and recommended approval of Change Order #2 for the 2014 North 18th Street Curb Replacement. The paperwork submitted to the Board shows a net change in the amount of $2,328.97 which needs to be changed to $2,328.95. The change order is a decrease in the amount of $2,328.95 which brings the revised contract amount to $151,829.30. Mr. Shriner moved for approval. Mrs. Ross seconded. Brian Beeler, Water Pollution Control, presented to the Board and recommended approval of a Significant Industrial user Permit for Caterpillar Inc. Mr. Beeler stated that Caterpillar is renewing a 4 year permit through the pre-treatment program.. Mrs. Ross moved for approval. Mr. Shriner seconded. Passed. CLAIMS Mike Jones, Controller, presented for Board approval, Claims in the amount of $5,393,058.02. Mrs. Murray asked if there were any further questions and there were none. Mrs. Ross moved for approval. Mr. Shriner seconded. Passed. MISCELLANEOUS Proclamation-Mental Illness Awareness Week Mayor Roswarski read the following proclamation: Mental Illness Awareness Week 2014 WHEREAS one in four adults experience mental illness every year and approximately half of chronic mental illness begins by the age of 14 and three-quarters by age 24. WHEREAS, mental illness include major depression, bipolar disorder, schizophrenia, anxiety disorders, posttraumatic stress disorder (PTSD), attention-deficit hyperactivity disorder (ADHD) and borderline personality disorder; and WHEREAS, mental illness is a highly treatable medical illness affecting the brain and requiring the same concern as cancer, heart disease, diabetes and other illnesses; WHEREAS, scientific research is constantly working toward breakthroughs in the understanding of mental illness, resulting in more effective treatments to allow people to reclaim full and productive lives, WHEREAS, misunderstandings exist about mental illness and social culture often wrongly imposes a stigma on mental illness that discourages people from seeking help when they need it; and WHEREAS every citizen and community can make a difference in helping to improve the lives of individuals and families affected by mental illness; NOW, THEREFORE BE IT RESOLVED, I Tony Roswarski, Mayor of Lafayette, Indiana do hereby proclaim October 5 through October 11, 2014 as Mental Illness Awareness Week in Lafayette, Indiana to increase public awareness of mental illness and to promote treatment and recovery. Ken Ritchey, NAMI Representative, thanked the Mayor and the Board for the proclamation and spreading awareness for mental health. Mr. Ritchey gave a brief description of the different types of mental health and the importance of the awareness.
