BOARD OF PUBLIC WORKS AND SAFETY MINUTES November 27, 2018 Regular Session: The Board of Public Works and Safety met in regular session on Tuesday, November 27, 2018 at 9:00 a.m. in the Common Council Chambers. Members present were: Gary Henriott, Cindy Murray, Norm Childress, Amy Moulton and Ron Shriner Jacque Chosnek, 1st Deputy City Attorney, was also present. President Henriott called the meeting to order. The Pledge of Allegiance was given to the flag of our Country. MINUTES Mrs. Moulton moved for approval of the minutes from the November 20, 2018 regular meeting. Mrs. Murray seconded. Passed. NEW BUSINESS Engineering Addendum #4-Utility Service Agreement for Raintree Medical Park Bob Foley, Engineering, presented to the Board and recommended approval of Addendum #4 for the Utility Service Agreement for Raintree Medical Park/Gary W. Schroeder Enterprises. They have submitted a check in the amount of $8,441.00 for expenditure reimbursement. Mr. Childress moved for approval. Mrs. Murray seconded. Passed. Change Order #1-Manufactirer’s Drive New Road Construction Jeromy Grenard, Public Works Director, presented to the Board and recommended approval of Change Order #1 for the Manufacturer’s Drive New Road Construction with Milestone Contractors. The change order reflects changes in scope and conditions. The change order is in the amount of $13,811.02 which brings the revised contract amount to $1,347,811.02. Mr. Shriner moved for approval. Mrs. Moulton seconded. Passed. Change Order #1-Breckenridge Street Reconstruction & Crosser Sports Complex Parking Lot Addition Mr. Grenard presented to the Board and recommended approval of Change Order #1 for the Breckenridge Street Reconstruction & Crosser Sports Complex Parking Lot Addition with Milestone Contractors. The change order reflects a change in scope and conditions. The change order is in the amount of $7,678.70 which brings the final contract amount to $704,678.70. Mrs. Moulton moved for approval. Mr. Childress seconded. Passed. Board of Public Works and Safety November 27, 2018 Page 2 Fire Department Supplemental #5-Fire Contract for Unincorporated Area of Fairfield Township Fire Chief, Richard Doyle, presented to the Board and recommended approval of Supplemental #5 for the Fire Contract for Unincorporated Area of Fairfield Township. This agreement is in the amount of $240,000.00 and helps assist with the storage building project and the expansion of the fire training facility. Mr. Childress moved for approval. Mrs. Murray seconded. Passed. Lafayette Housing Authority Agreement between Lafayette Housing Consortium and YWCA of Greater Lafayette-Fresh Start Program Tenant Based Rental Assistance 2018-2019 Valerie Oakley, Project Manager, presented to the Board and recommended approval of an Agreement between Lafayette Housing Consortium and YWCA of Greater Lafayette-Fresh Start Program Tenant Based Rental Assistance 2018-2019. This program will provide up to 12 months of rent assistance for victims of domestic violence that are participating in the YWCA-DVIPP Program. This agreement is in the amount of $42,000.00. Mr. Childress moved for approval. Mr. Shriner seconded. Passed. CLAIMS Tim Clary, Deputy Controller, presented for Board approval, Claims in the amount of $7,656,357.64. President Henriott asked if there were any further questions and there were none. Mrs. Murray moved for approval. Mrs. Moulton seconded. Passed.
