MINUTES
November 28, 2023
Regular Session:
The Board of Public Works and Safety met in regular session on Tuesday, November 28, 2023 at 9:00 a.m. in the
Common Council Chambers. Members present were: Gary Henriott, Cindy Murray, Amy Moulton, and Ron
Shriner. Absent: Eric Rody
Jacque Chosnek, City Attorney, was also present.
President Henriott called the meeting to order.
The Pledge of Allegiance was given to the flag of our Country.
MINUTES
Mrs. Moulton moved for approval of the minutes from the November 21, 2023 regular meeting. Mrs. Murray
seconded. Passed.
Proposal Opening
Proposal-Offer Certain Real Estate for Sale to an Abutting Landowner (N. 26th Street)
This being the time set to open proposals for the Proposal-Offer Certain Real Estate for Sale to an Abutting
Landowner (N. 26th Street) and the following proposals were received and opened:
Iron Men Properties of Lafayette
Lafayette, Indiana
Proposal: $14,700.00
Mr. Shriner moved to take the proposals under advisement for further review. Mrs. Moulton seconded. Passed.

Glick Wellfield Improvements
President Henriott stated that this item will remain under advisement.
NEW BUSINESS
Engineering
Change Order #3-Kossuth Street Resurface from 4th Street to Earl Avenue
Jeromy Grenard, Public Works Director, presented to the Board and recommended approval of Change Order #3
for the Kossuth Street Resurface from 4th Street to Earl Avenue that will reconcile quantities and close out the
contract. The change order is a decrease in the amount of $149,185.00 which brings the revised contract amount
to $1,561,641.56. Mrs. Murray moved for approval. Mrs. Moulton seconded. Passed.

Certificate of Final Completion-Kossuth Street Resurface from 4th Street to Earl Avenue
Mr. Grenard presented to the Board and recommended approval of a Certificate of Final Completion for Kossuth
Street Resurface from 4th Street Earl Avenue that includes final completion, final payment and release of retainage
to Midwest Paving, LLC. The contractor had satisfactorily completed all work items for the project as of
November 10, 2023 and all applicable inspections have been passed by the Engineering Department. Mr. Shriner
moved for approval. Mrs. Moulton seconded. Passed.
Permission to Advertise-Utility Emergency Repairs Program 2024
Mr. Grenard presented to the Board and recommended approval of a Permission to Advertise for the Utility
Emergency Repairs Program 2024. The publications are set to run on December 1 & 8, 2023 with the bid opening
on December 19, 2023. Mrs. Murray moved for approval. Mrs. Moulton seconded. Passed.
Certificate of Completion-S. 24th Street Concrete Project
Mr. Grenard presented to the Board and recommended approval of a Certificate of Completion for the S. 24th
Street Concrete Project with Dixon Construction. Mr. Grenard stated that the work has passed all inspections, no
outstanding work items and all final pay requests have been submitted. The 1-year warranty period begins today.
Mrs. Moulton moved for approval. Mr. Shriner seconded. Passed.
Certificate of Completion-Miscellaneous Projects-1
Mr. Grenard presented to the Board and recommended approval of a Certificate of Completion for the
Miscellaneous Project-1 with Dixon Construction. Mr. Grenard stated that the work has passed all inspections, no
outstanding work items and all final pay requests have been submitted. The 1-year warranty period begins today.
Mr. Shriner moved for approval. Mrs. Moulton seconded. Passed.
Certificate of Completion-O’Farrell Road Concrete Replacement Project
Mr. Grenard presented to the Board and recommended approval of a Certificate of Completion for the O’Farrell
Road Concrete Replacement Project with Dixon Construction. Mr. Grenard stated that the work has passed all
inspections, no outstanding work items and all final pay requests have been submitted. The 1-year warranty
period begins today. Mr. Shriner moved for approval. Mrs. Moulton seconded. Passed.
Certificate of Completion-Highland Park Curb Replacement Project
Mr. Grenard presented to the Board and recommended approval of a Certificate of Completion for the Highland
Park Curb Replacement Project with Dixon Construction. Mr. Grenard stated that the work has passed all
inspections, no outstanding work items and all final pay requests have been submitted. The 1-year warranty
period begins today. Mrs. Murray moved for approval. Mrs. Moulton seconded. Passed.
Change Order #1-Vinton Woods Concrete Replacement Project
Mr. Grenard presented to the Board and recommended approval of Change Order #1 for the Vinton Woods
Concrete Replacement Project with Heartland Contractors. The change order includes the removal and
replacement of concrete pavement. The change order is an increase in the amount of $4,424.88 which brings the
revised contract amount to $231,758.46. Mrs. Murray moved for approval. Mrs. Moulton seconded. Passed.

Change Order #1-Valley Forge Concrete Replacement Project
Mr. Grenard presented to the Board and recommended approval of Change Order #1 for the Valley Forge
Concrete Replacement Project with Dixon Construction. The change order includes the addition of ramps,
sidewalks and miscellaneous concrete transitions. The change order is an increase in the amount of $28,017.50
which brings the revised contract amount to $365,334.50. Mr. Shriner moved for approval. Mrs. Moulton
seconded. Passed.
CLAIMS
Jeremy Diehl, Controller, presented for Board approval of Claims in the amount of $607,990.85. President
Henriott asked if there were any further questions and there were none. Mrs. Murray moved for approval. Mrs.
Moulton seconded. Passed.
MISCELLANEOUS
Special Event Request-Pearl Harbor Day Ceremony
Robert Mantle, Event Representative, presented to the Board and recommended approval of a Special Event
Request for Pearl Harbor Day Ceremony to be held on December 7, 2023 from 11:00am-2:00pm on the John T.
Myers Bridge. Mrs. Moulton moved for approval. Mrs. Murray seconded. Passed.
Mrs. Murray reminded everyone that the Christmas Parade will be held this Saturday, December 2, 2023 starting
at 5:30pm in Downtown Lafayette.
