Tabled until requested information is received. B. Application from Warren Knight, Police Officer, for a normal Police retirement eff. 3/17/2012. B. Application from Thomas Blake, Custodian (BOE), for a General City service-connected disability retirement, eff. 2/6/2012. C. Request for a rollover of pension contributions plus interest from Gayle Wentworth, who was laid-off effective 6/30/2003 in the amount of $4,548.59. D. Pension calculation for Joseph Novak, who retired 1/21/2012. Mr. Novak’s pension is calculated for an annual amount of $25,596.21. Mr. Novak’s application for retirement was approved at the 1/21/2012 meeting. E. Pension calculation for Annette Dubicki, who retired 1/2/2012. Ms. Dubicki’s pension is calculated for an annual amount of $7,090.15. Ms. Dubicki’s application for retirement was approved at the 12/20/2011 meeting. F. Pension calculation for Teresa Chmelecki, who retired 1/18/2012. Ms. Chmelecki’s pension is calculated for an annual amount of $30,568.38. Ms. Chmelecki’s application for retirement was approved at the 12/20/2011 meeting. G. Pension calculation for Thomas Trasko, who retired 2/1/2012. Mr. Trasko’s pension is calculated for an annual amount of $56,613.53. Mr. Trasko’s application for retirement was approved at the 1/17/2012 meeting. H. Pension calculation for John Flaherty, who retired 1/6/2012. Mr. Flaherty’s pension is calculated for an annual amount of $13,013.73. Mr. Flaherty’s application for retirement was approved at the 12/20/2011 meeting. I. Pension calculation for Gary Bohara, who retired 1/7/2012. Mr. Bohara’s pension is calculated for an annual amount of $67,612.78. Mr. Bohara’s application for retirement was approved at the 12/20/2011 meeting. J. Pension calculation for Douglas Gardner, who retired 1/7/2012. Mr. Gardner’s pension is calculated for an annual amount of $24,498.83. Mr. Gardner’s application for retirement was approved at the 12/20/2011 meeting. K. Notification of the death of retiree Paul Koziol. Mr. Koziol retired from NPU on 2/28/1985, and passed away on 12/14/2011. Notification of survivorship pension benefit for Anne Koziol in the annual amount of $10,279.98. L.
