CALL TO ORDER Chair Cooper called the meeting to order at 5:01 p.m. 1.A ROLL CALL Present: 5 - Chair Jim Cooper, Vice Chair Mark Hansen, Boardmember Larry Dibble, Boardmember Cheryl Selby and Boardmember Russ Gies 2. APPROVAL OF MINUTES 2.A 22-0839 Approval of April 11, 2022 LEOFF Board Minutes The minutes were approved. 3. BUSINESS ITEMS 3.A 22-0840 Approval of Case #22-5 Hearing Aid Request The decision was discussed and continued to the 11/14/22 LEOFF I Disability Board meeting. 3.B 22-0841 Approval of Case #22-6 Medical Test Request Boardmember Dibble moved, seconded by Vice Chair Hansen, to approve payment for a medical test not covered by insurance in the amount of $118.00. The motion carried by the following vote: Aye: 5 - Chair Cooper, Vice Chair Hansen, Boardmember Dibble, Boardmember Selby and Boardmember Gies 3.C 22-0934 Approval of Case #22-7 Long Term Care Request Boardmember Dibble moved, seconded by Boardmember Selby, to approve payment for long-term care for a retired LEOFF 1 member up to the allowable amount equal to a semi-private room at Panaroma City per the LEOFF policy.
